By accessing or using the Platform, you acknowledge that you have read and agree to this Privacy Policy and the ClearDiagnosis Terms of Use, and you consent to the collection, use, storage, and disclosure of information as described herein.

We may collect information you provide directly through the Platform, including personal information such as your name, email address, phone number, mailing address, account registration information, payment and billing information, communications submitted through the Platform, support requests, and other information you choose to provide. We may also collect health-related information submitted for physician review services, including medical records, laboratory reports, imaging, questionnaires, intake forms, health histories, and other related information. Information may also be submitted by authorized representatives, caregivers, or legally authorized individuals acting on behalf of a user.
When you use the Platform, we may automatically collect certain technical and usage information, including IP address, browser type, device information, operating system, access dates and times, referring URLs, pages viewed, interaction data, log information, and usage analytics. This information may be used for Platform functionality, security, analytics, quality assurance, and operational purposes.
4. How We Use and Share Information

4A. Mobile Information and SMS Messaging for MFA
If you provide your mobile phone number and opt in to receive SMS or text messages from ClearDiagnosis for multi-factor authentication (“MFA”), we may collect and process information related to the MFA SMS program, including the mobile phone number you provide, the date, time, and method by which you opted in, your messaging preferences and opt-out status, message content and delivery metadata (such as timestamps and delivery status), and any responses you send (for example, STOP or HELP).
We use this information solely to deliver MFA verification codes you have requested, to honor your opt-in and opt-out preferences, to maintain records of consent as required by law, to support, secure, and operate the MFA SMS program, and to comply with applicable telecommunications, consumer protection, and privacy laws. ClearDiagnosis does not send SMS messages for marketing, appointment reminders, case updates, physician communications, or other promotional or service purposes.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors and service providers in support of the services we provide to you (for example, telecommunications and SMS delivery providers, customer support, security, and fraud prevention providers) is permitted solely to operate and support the MFA SMS program. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

5. Use of Artificial Intelligence and Automated Tools
ClearDiagnosis may use artificial intelligence (“AI”) and other automated technologies to assist in organizing, analyzing, summarizing, or presenting information submitted through the Platform. These technologies are used to support physician review services and platform functionality. AI tools do not independently provide medical opinions or medical care.
Information submitted through the Platform may be processed using such technologies as part of the services provided.
6. How We Protect Your Information
We implement reasonable administrative, technical, and physical safeguards designed to protect information submitted through this website from unauthorized access, use, or disclosure.
To the extent ClearDiagnosis or affiliated physician providers are subject to HIPAA and applicable state privacy laws, protected health information will be handled in accordance with those laws and applicable Notices of Privacy Practices.
While we implement reasonable safeguards designed to protect your information, no method of electronic transmission or storage can guarantee absolute security. However, no method of transmission over the Internet or electronic storage is completely secure. By using our Services, you acknowledge and accept this risk.

You have the right to request access to or a copy of the information we hold about you, as well as to request corrections to any inaccurate or incomplete information. You are also welcome to ask questions at any time about how your information is used or stored.
